Before you start
Have the following handy before you fill in the form:
- The participant's full name and NDIS number
- Your plan management type (self-managed, plan-managed, or agency-managed)
- The participant's date of birth (required for agency-managed claims only)
- Your address, mobile number, and email address
What happens next
Once submitted, your invoice and supporting documents will land in your inbox. If you're plan-managed, forward them to your plan manager. If you're self-managed, submit via your NDIS portal. If you're agency-managed, we will submit the claim for you. We will be in touch with you to confirm some information before we submit the claim. Agency-managed claims are usually paid within a few days, but please allow up to six weeks for the NDIA to process the invoice. This is outside our control, but we'll keep you updated.
